Appointment Cancellations

This office does charge a fee for missed appointments.  Failing to show up for an appointment without notifying our office at least 24-hours prior to your appointment or cancelling an appointment with less than a 24-hour notice will result in a charge of $25 being placed on your account.  It is the policy of this office not to schedule a patient that has two recorded “no show” appointments without first securing a deposit.

Worker’s Compensation
NewView welcomes patients who have been injured on the job and are covered by worker’s compensation. To avoid paying for visit at time of service, patient must have the following information:

  • Company Name
  • Company Responsible for Payment (if different than above)
  • Date of Injury
  • Claim Number
  • Human Resource Contact Name
  • Human Resource Phone Number
  • Human Resource Fax Number

PLEASE NOTE: PATIENT IS RESPONSIBLE FOR PAYMENT IF CLAIM IS DENIED.

Prescription Refills
Please allow 24 hours’ notice for prescription refills.  You may have your pharmacist either fax your request or call our office.  Some medications require pre-authorization through your insurance company and special forms.  Please have your insurance company fax those forms so that we can submit them for you within 3-5 working days. Prescription refills cannot be given for new eye problems without being diagnosed by the doctor.   Refills requested outside of the time of your visit are subject to processing fees.

Medical Records Release Forms
Please allow ten to fifteen days for the receipt of medical records. Due to increased demands for, and costs of, paperwork in a medical office, a fee of $25 is charged for preparation, copy, and transfer of any medical records. Additional fees may apply if number of pages to be reproduced exceed thirty (30) or if a formal, written medical report is requested.

Completion of Medical Forms
Please allow three to five working days for the completion of medical forms. The completion of medical forms are subject to processing fees.

Dr. Griffiths has surgical privileges at Reston Ambulatory Surgery Center and hospital privileges at Reston Hospital Center,

Seminars
Dr. Griffiths donates her time to provide educational seminars to the community to heighten awareness for early detection of eye disease.  She lectures on the “Aging Eye” and Eye Safety to local businesses and community centers.  She also provides noon time seminars at local companies on the latest in refractive surgery options.

Insurance Coverage
We accept most medical and vision insurance plans.  Your insurance coverage is an agreement between you and your insurance company.  Current insurance information and correct addresses are imperative for prompt claim reimbursement.  Co-pays are due at the time of the visit. Most insurance companies do not pay for refractions, (measurement for glass prescriptions).  You may be required to pay additional fees for this service at the time of your visit.  Please direct any billing inquiries to the office manager at 703-834-9777.
